Your referral of your family and friends is the best compliment you could give us. We want to thank you with the Eye Care, Eye Share Referral Program. The Eye Care, Eye Share Referral Program is a program designed to ensure your family and friends have the same comprehensive eye care that you receive from Clear View Optometric.

WHAT ARE THE BENEFITS?

As a thank you for your referral, you will receive a $20 eyewear credit for every new patient that you refer. The patient you refer will also receive a $20 eyewear credit.

WHAT CAN THEY BE REDEEMED FOR?

The $20 eyewear credit can be used towards your NEXT eyewear purchase, and your friend or family members 1st purchase. They can be used toward the purchase of a complete pair of eyeglasses or a year supply of contact lenses. Credit can not be used towards services or copays. Eyewear credits do not have any monetary value.

IS THE CREDIT TRANSFERABLE?

The $20 eyewear credit is NOT transferable between patients.

HOW DO WE TRACK 20/20 REFERRALS?

When a referred patient comes to their first appointment they MUST present us with a referral certificate that is filled out with their name and your name and date of birth or phone number. You will receive a certificate in the mail for your next eyewear purchase or annual supply of contact lenses.

HOW MANY REFERRAL CREDITS CAN BE EARNED?

There are no limits as to how many patients you can refer.

DO THE CREDITS EXPIRE?

Yes the credit expires 1 year after issue date
